You will be joining the fantastic Chronic Care team and provide culturally safe, evidence-based diabetes education and support across remote Northern Territory communities. Job Details: Permanent full time position Base salary of up to $113,669.57 plus 12% superannuation and on top of this salary packaging available for a big tax saving Relocation assistance available Travel allowances provided Professional development opportunities available Location: Based in the Northern Territory with regular travel to remote community health centres. Your Role: Provide individual and group diabetes education and support for clients and families Collaborate with health centre staff and specialists to deliver integrated chronic disease care Offer clinical leadership and mentoring to health professionals in diabetes management Promote evidence-based standards and culturally appropriate care Travel weekly to remote communities to deliver education and monitor patient progress Maintain accurate clinical records and contribute to reporting and quality improvement initiatives To be eligible you must have: Full AHPRA registration with no restrictions Be a credentialed Diabetes Educator Extensive experience in diabetes and chronic disease management Strong communication and interpersonal skills Ability to work in culturally safe ways within Aboriginal communities Current manual driver’s licence and First Aid Certificate Current OCHRE clearance and acceptable vaccination status Right to work in Australia To find out more about this position or to receive a copy of the job description please email emily@curis.com.au and a consultant will be in touch within 24 hours.
